Introduction:
Pharmacogenomics (PGx) is redefining how medications are prescribed, dispensed and monitored, ushering in an era of truly personalised medicine. However, its clinical and system-level impact depends on how effectively genomics insights are embedded within integrated digital care pathways that support continuity of care, equity, and sustainable health system transformation.
This joint Technology Advisory Group-Hospital Pharmacy session along with the Personalised and Precision Medicine SIG demonstrates how pharmacists can translate pharmacogenomic data into actionable, technology-enabled clinical decisions.
In addition, the session will critically acknowledge current limitations in pharmacogenomics adoption, including variability in the strength of evidence, workforce readiness, data interoperability, cost, infrastructure constraints at low- and middle-income countries and resource-variable healthcare settings.
To enhance applicability and scalability, the session will introduce practical implementation frameworks aligned with existing FIP digital health and precision medicine initiatives.

1. Understand the foundations, benefits, and clinical value of pharmacogenomics and how digital systems enable its translation into personalised patient care.
2. Demonstrate how pharmacogenomic data can be integrated into digital health systems for real-time effective and optimised therapeutic decision-making.
3. Explore strategies for scaling digital pharmacogenomics through collaboration, workforce development, and system integration.
Take home messages:
Digital pharmacogenomics bridges genetics and care. Integration of AI-based decision support into the prescribing process within pharmacies could enable pharmacists to provide safer, more individualised treatment. Integrating genomic data with digital workflows improves accuracy, enhances patient engagement, and strengthens clinical governance, empowering pharmacists to lead the transformation toward precision, equitable health care.
