This full-day programme will introduce participants to the structure, scope, and innovations of hospital pharmacy practice in Canada. It will focus on interprofessional models of care, medication-use system design, and safe, equitable access to therapies. The session will begin with a plenary overview delivered at the Centre hospitalier de l’Université de Montréal (CHUM), highlighting national trends and insights into the advanced clinical practice of hospital pharmacists in Canada and Quebec.
Participants will then engage in guided site visits at three rebuilt teaching hospitals in Montreal, each showcasing distinct approaches to clinical practice, automation, compounding, quality systems, and integration with academic and research programs. The session aims to provide a comprehensive understanding of Canadian hospital pharmacy practice and the strategic priorities shaping the profession.
Learning objectives:
By the end of this session, participants will be able to:
- Describe the current landscape of hospital pharmacy practice in Canada, including system-level priorities, models of care, and workforce considerations.
- Compare different approaches to service delivery, automation, clinical practice, and governance across three major academic hospital centres.
- Identify key innovations and quality improvement strategies that support safe and effective medication-use systems in large health-system environments.
- Discuss how Canadian hospital pharmacy practices align with global trends and contribute to advancing patient safety, operational excellence, and interprofessional collaboration.
